• Appointment required: please register at https://bit.ly/2021UKCDflu
  • Openings available on Oct. 21: 9 am-3:50 pm
  • Appt. will take 10 minutes
• Must bring your UK ID.
• Please complete and print consent form prior to arriving (top box, middle box, sign, and date).

• Enter the Wellness Clinic via MN 328.
  • Exit via DMD Student Clinic reception lobby.
• Will receive a UK flu badge buddy at appointment.
• Paperwork will be e-submitted following appointment for inclusion in UK record.
• Standard flu vaccine only (no high-dose available).
